Clinical Improvement in Painful Diabetic Peripheral Neuropathy Following Ayurvedic Management: A Case Report

Abstract

Painful diabetic peripheral neuropathy (DPN) frequently compromises comfort and function in people with diabetes, and symptoms may continue despite conventional medicines. We report the course of a 58-year-old man with a 20-year history of type 2 diabetes mellitus who experienced burning and tingling in all four limbs while receiving standard treatment. Neurological assessment demonstrated distal sensory impairment in a glove-and-stocking pattern, weak ankle reflexes, and reduced appreciation of vibration. The clinical diagnosis was DPN, interpreted as Vatarakta within the Ayurvedic framework. Management comprised Shalishastika Pinda Swedana, Saghrita Panchatikta Basti, and oral Ayurvedic preparations. By the end of two months, the modified Neuropathy Disability Score had fallen from 8 to 1; pain and daily functioning had also improved markedly. The observed clinical response supports evaluation of this combined regimen in controlled studies.
